Instructions
We have 3 offices serving the Long Island and Queens area. Please call ahead to schedule an appointment and for fee information. Our contact information is located on the Contact page.
Items to bring to the examination include:
- INS Form I-693 (you can download the form here)
- Your personal identification with a photo on it and your passport.
- Your medical records and a copy of all the vaccinations that you have received since birth.
We have all necessary forms available at our offices. If and when your INS Form I-693 is certified by Dr. Han after your examination, two copies of the completed forms will be mailed to you. One copy will be your applicant's copy, and the other will be the USCIS copy. The USCIS copy will be in a separate sealed envelope. Do not open this envelope, doing so will void your certification. To assure faster certification of your forms, be sure to bring all the required items listed above.
